Davenport Community School District Health Services

HEALTH AND EDUCATION—–PARTNERS IN SUCCESS

Davenport Community Schools is committed to providing a school environment that enhances learning and development of lifelong wellness practices. We believe well-being has a direct impact on student learning and behavior, therefore, the District will develop administrative regulations using standards designed to promote wellness, as mandated by federal and state law. To view these guidelines, please visit our Wellness Policy.

Introduction to Health Services

The focus of the Davenport Community Schools Health Services Program is the promotion of an optimum state of health for the school age child through preventative measures such as health screenings, health assessments, health education, health counseling and referrals. The program is based on the knowledge that a healthy body, healthy mind, and healthful living habits are essential components in the student’s quest to achieve his/her maximum educational potential.

A staff of professional nurses as well as paraprofessional staff provides health services to the students.

To provide the safest and best care to all students in the Davenport Community School District (DCSD), it is important that the school has the most up-to-date health information; including addresses and phone numbers. Please contact your child's school office when changes occur with your student’s health, addresses, emergency contacts or phone number.

Many of the forms necessary to provide safe care and promote health in your student are available on this web page.  Please feel free to print, complete, and either send the form back to school with your student or complete the form online and email it back to the school nurse as an attachment. Students who attend CHC and Vera French will need to complete the specific agency’s medical release form for the medical information to be shared with the school health office personnel.

Information provided to the school regarding the health and care of students may be shared with school personnel for the provision of appropriate health and /or educational services.

  • Medicaid: 

  • Go to https://dhsservices.iowa.gov/apspssp/ssp.portal or call 1-855-889-7985. You qualify if your incomes is between 0% and 138% of the Federal Poverty Level. For the individual that would be up to $16,104.60 and for a Family of Four- $32,913.
  • Healthy & Well Kids in Iowa (hawk-i): 

  • Go to:https://dhs.iowa.gov/hawki .  Hawk-i is a state program which offers low-cost or no-cost health and dental insurance for children in families with incomes too high to qualify for Medicaid, but too low to afford private family coverage. This coverage is available to kids and teens ages 1-19, who live in Iowa.  Applications are available upon request at the school or contact Care for Kids at Scott County Health Department at or call (563) 328-4114.
  • Marketplace: 

  • Go to www. healthcare.gov or call 1-800-318-2596.  Penalty: Penalties are taken directly out of federal tax returns if you qualify for coverage but choose not to enroll.

Absences: Students are required by law to attend school. If your child must be absent from school, please notify the school office with two symptoms as early as possible on the morning of the absence. Serious illnesses that may require a long-term absence must be verified by a medical doctor. Home Instruction is available for students on written request of the attending physician.
