The focus of the Davenport Community Schools Health Services Program is the promotion of an optimum state of health for the school age child through preventative measures such as health screenings, health assessments, health education, health counseling and referrals. The program is based on the knowledge that a healthy body, healthy mind, and healthful living habits are essential components in the student’s quest to achieve his/her maximum educational potential.
A staff of professional nurses as well as paraprofessional staff provides health services to the students.
To provide the safest and best care to all students in the Davenport Community School District (DCSD), it is important that the school has the most up-to-date health information; including addresses and phone numbers. Please contact your child's school office when changes occur with your student’s health, addresses, emergency contacts or phone number.

Many of the forms necessary to provide safe care and promote health in your student are available on this web page. Please feel free to print, complete, and either send the form back to school with your student or complete the form online and email it back to the school nurse as an attachment. Students who attend CHC and Vera French will need to complete the specific agency’s medical release form for the medical information to be shared with the school health office personnel.
Information provided to the school regarding the health and care of students may be shared with school personnel for the provision of appropriate health and /or educational services.

Absences: Students are required by law to attend school. If your child must be absent from school, please notify the school office with two symptoms as early as possible on the morning of the absence. Serious illnesses that may require a long-term absence must be verified by a medical doctor. Home Instruction is available for students on written request of the attending physician.
